There are two main reasons this can happen:
- If you’ve upgraded SSMS, you’ll need to uninstall and then reinstall SQL Prompt so it can integrate with the new version, this won’t affect your configuration files, history or settings.
If you've upgraded or reinstalled SQL Prompt and it’s no longer showing on the toolbar, go to Extensions > Customize Menu > Extensions Menu and uncheck both SQL Prompt options, then Save and Restart.

SQL Prompt should then show in the toolbar.
