Ended up bouncing the server that SQL Monitor is on and all is good now.
The question becomes what type of errors are you getting? Is there a pattern to day of the week, time of day, specific servers? Could it be a network issue? A switch issue? We had this type of senario a few years back where SQL Mon kept telling us that a few of our SQL Servers were up and down over and over Fri at 10PM. After the pattern of two weeks in a row we got the network team involved and they found the issue with a switch going bad. / comments

UPDATE: We evicted the down server from the AG group and then SQL Monitor was able to monitor the 3 active servers just fine. / comments

We are currently monitoring 11 different SQL Servers. The number of userids with 'sa' rights is different for all 11. So, I need to set the threshold different for each of them. / comments

At the current time I cannot upgrade to 10.1.8 as we just upgraded to 10.1.14. However, I will make a note of that and come back around to visit the upgrade. / comments

Can you please double check that it is disabled for all the desired level(s)? It is. However, Friday I was making changes to that and I think I may have stopped them from being raised via that. I will watch it and let you know if they keep coming. But, it does look like it has stopped them.

Have you changed the rights to the ID that connects to SQL Server? Is the ID locked, does the ID have a bad password or the password changed but not updated within SQL Monitor connection string? The every 5 minutes could be a default retry time. Meaning, it fails to connect and will automatically try and connect again 5 minutes later. / comments
