Would it be possible to add this as a feature request for version 9?
Being able to say include extra sql from a given file in the Synchronization Wizard would be be extremely helpful. Especially if the file path for the extra sql could then be saved in the project.

Your assumption is very likely correct. I was successful in my attempts to reproduce similar behavior in Notepad.
Here is my Notepad test:
Open Notepad.
Type something.
File->Exit.
Press and release ALT when asked to save the file.
Press Y. Nothing happens.
Press Y again. The Save As dialog comes up.
If ALT is not pressed initially then Y immediately brings up the Save As dialog, similar to pressing B or N in the Synchronization Wizard triggering the Back and Next buttons now.

I am running under 32 bit XP Pro with Service Pack 3. I have both .NET 1.1.4322 and 2.0.50727 installed.
I can reliably reproduce this with the following:
Open the Synchronization Wizard using the Management Studio 2005 radio selection Application Options.
Click Next.
Press and release ALT. The shortcut line should appear under B on the Back button.
Without any further use of ALT, now press B. At this point nothing happens for me. If I press B again then the wizard moves back one step.

With the way I use SQL Compare, pre selecting everything is a great aid. I have multiple development servers to keep in line for nightly builds with the team.
Since there is strong demand for both default select all and select nothing I can get behind the idea of adding this to the application options. I won't object too much if the default is select nothing and I have to go into options once and check the select all checkbox. So long as there remains a way to default select everything I'm happy. Could you add a checkbox to the sync wizard which behaves similar to the "Close dialog box on completion" when a comparison is run? Could that checkbox then save its state as the close on completion box does? This could satisfy both groups of users. / comments

I can say that I have used the self comparison option to go through my database in the past. Personally I would not object (call this one indifference to) on save or creation of a project to throw a dialog confirming comparison to self, however throwing that dialog on each comparison run would become quite annoying. / comments

I'd guess that less than 50% of the time the last used comparison is the one I'm going to use when I crack out SQL Compare the next time. I am also the sole DBA in charge of comparisons.
Personally, I would prefer a checkbox in the application options that works along the lines of "open last project on startup" When not checked SQL Compare starts and looks as though I just canceled out of the comparison dialog, allowing me to use the 6 (plus or minus as defined by another variable in application options?) last used list to find the project I need. / comments

This could work. It might work better than creating one time use projects.
I do notice that if I change either of the servers or databases being compared that the title bar does not update to reflect that change. Even after a "Save As". Only after a comparison is run and "Edit Project" is selected, or the file is re-loaded does the title bar reflect the server/database changes that have been made.
Say I have a project comparing server.development v server.archive. If I change this to server.development v server.production the title bar still reflects the server.development v server.archive operation. This has the potential to be a bit confusing when glancing at the project window. / comments

Another option would be to have the open list act more like a favorites/bookmarks list from your web browser of choice. I have some projects that I rarely use and some that I use nearly daily.
Under version 7 those daily used projects naturally floated up to the top but could be fun to track down if I used one of my rarely used projects and the list order changed from what I'm used to on a day to day basis.
A fixed list would be preferable for my usage with the option to open other projects as needed. / comments
