Having the same problem. Still see SQL Prompt in _other_ versions of SSMS, but history/prompt are gone from SSMS v22.
I had SQL Prompt v11.2.0 working w/ SSMS v22. I had an “update available”, launched it, quit out of all SSMS/VS windows, let the update continue, then restarted SSMS and no longer have the menu or anything in Extensions for SSMS v22.

I tried running
"C:\Program Files\Microsoft SQL Server Management Studio 22\Release\Common7\IDE\SSMS.exe" /resetsettings
That has cleared up the extension cache in the past, but doesn't seem to have helped at all this time. -

I was able to fix this by uninstalling and reinstalling prompt 11.2.1.20094

Where can I get the latest version for SQL Prompt.

I recommend the following if you've installed SQL Prompt and you don't see it in the top menu bar:
- Go to the Extensions menu in the top menu bar:
- Choose Customize Menu
- Choose the Extensions Menu tab
- If you see SQL Prompt in the list, uncheck the boxes. Then click the Save and Restart button.
If you DO NOT see SQL Prompt in the list, do the following:
- Uninstall SQL Prompt
- Reinstall SQL Prompt
- Restart your machine
- Start SQL Server Management Studio
- Try the steps at the top of this comment

My SSMS/Extensions was a completely blank screen - nothing at all shown.
But the bigger issue is that just clicking the “upgrade” option and letting the installer do the upgrade should not need us to uninstall/reinstall SQL Prompt.

After upgrading to version 11.2.1.20094 today from version 11.2.0.19838 I no longer can see prompt in SSMS 22. This was working fine for me in version 11.2.0.19838. How to fix?
