If you're on SSMS 19 preview 4 you can use the latest version of SQL Prompt:
https://download.red-gate.com/checkforupdates/SQLPrompt/SQLPrompt_10.13.4.32349.exe
If you're on a previous preview you'll need to downgrade to the previous version of SQL Prompt:
https://download.red-gate.com/checkforupdates/SQLPrompt/SQLPrompt_10.13.2.31952.exe

Just uninstalled/installed 10.13.7.360 of SQL Prompt 10, still not part of Visual Studio 2019. This happens on a regular basis.

The SQL Prompt menu only appears when a SQL Query file is opened in VS, please can you try this?

VS version is 16.11.23.
I noticed that SQL Prompt shows in the about Visual Studio window, but shows as 1.0.0.0.
Any ideas?
