Database and Disk Utilization Trending

I would like to see the tool to provide growth numbers and trending information for databases. The alert should notify based on threshold for reaching disk capacity by megabytes or percentage or by expected limitation based on current database growth. For example disk D:\ will run out of space within 30 days based on XXX database growth with link to database size history.
