This also happened to myself and 3 other developers. The SQL prompt menu is available and works in VS2015, but is not available in SSMS 2008 R2. The Red Gate menu is also greyed out... I've tried repairing the SSMS Integration Pack 1 and SSMS Integration Pack Framework 1 entries in the installed programs menu of Windows, but that did not have any effect.

Thanks for your post. Unfortunately SQL Prompt no longer supports SSMS 2008R2 and lower. You can upgrade your version of SSMS for free here

@James R I have SQL 2016 SSMS studio, I my menu is grayed out also. To be fair, my work station has 2008 r2 and 2014 - so perhaps that's where the failure comes from. -

SQL Prompt should install to all versions that it is able to be installed to. It being greyed out sounds like a licensing issue.
Are you able to check if Prompt is activated correctly? If you go to SQL Prompt- Help - About, can you see your serial number in the left bottom corner?
