This is the solution I found in the release notes regarding your problem:-
SQL Server Management Studio Specific Issues • When you uninstall SQL Prompt, the SQL Prompt menu is not removed from the SQL Server Management Studio toolbar. To remove the SQL Prompt menu, on the Tools menu click Customize. Drag the SQL Prompt menu to the SQL Server Management Studio title bar. Close the Customize dialog box. This is actually an issue with SQL Server Management Studio rather than SQL Prompt 3.

Hi gents,
Sorry about this. When you remove an add-in there's an method on one of the VSIP interfaces you have to call to remove items from the top-level menu. Unfortunately this method hasn't been implemented so it does precisely nothing and we still haven't found a workaround yet: it may be that there isn't one. Again, apologies. We are going to continue looking for a way around this problem so it's possible we might find a fix in time for the final release.

When I click on any of the commands, I get a message saying:
"The Add-in supporting this command could not be loaded. Do you wish to remove this command? If you choose yes, you can re-create the command by reinstalling the Add-in."
So I went through the commands, one-by-one, clicking on them and clicking "Yes" to remove them, and they disappeared...
But the top-level menus are STILL there, greyed-out! How do I get rid of them?? Restarting SSMS does not make them go away. I'm really displeased with this; there is little I dislike more than software that, when "uninstalled", leaves little landmines lying around that I need to manually clean up.
