I ran into something similar after updating SSMS, and in my case the issue was tied to extension compatibility after the upgrade. Reinstalling SQL Prompt and checking whether the latest version fully supports SSMS 21 helped. Also worth clearing old settings/cache if the menu still doesn’t appear.  / comments
