I installed SQL Prompt three times, twice from SQLToolbeltEssentials.exe and once using SQLPrompt_11.3.18.25519.exe. In none of these attempts did SQL Prompt appear on my SSMS (v22) Extensions menu. I got the following error on the third attempt:

Fixed by updating to SSMS v22.8 (SQL Prompt needs 22.3 or higher)
