Hi Dave,
SQL Server Data Tools must be installed from the Visual Studio Installer (Data storage and processing workload) for SQL Prompt to load in Visual Studio. However, you mention you are still working ok in VS 2019, for which the same applies.
If you'd like us to take a better look at what could be preventing SQL Prompt showing up, please raise a support ticket with us at support@red-gate.com -
Another uninstall/reinstall ended up working. SSDT was installed with the new VS2022 installation before SQL Prompt was reinstalled.

Is there some other method of adding the extension to a new VS instance?
