I have installed SQL Monitor 12 and everything seems to be working fine except when I try to change the Authentication Method to Active Directory. When Add a domain the web app hangs when I click Save Settings and nothing happens. Clicking the test connection button works fine. I have followed all the provided documentation provided but nothing seems to work.

Can I ask which specific version of SQL Monitor 12 you have installed? You can find it in the about section in the configuration tab.
Can you try doing the same thing, but in a different browser/in an incognito window? -
Version is 12.0.16.22039. I have been using Chrome but will try in incognito tab. Same issue exists when using Chrome in incognito name

Is it possible for you to try and upgrade SQL monitor to see if that resolves the issues? I haven't seen the setup section hang before, so I can't say for certain what it would be. If an upgrade doesn't fix it, then a look into the logs might be required to see what's happening.

How is the website communicating with AD? If it is communicating via the base monitor, would the account running the base monitor need to be able to delegate permissions to access AD?

It would be communicating via LDAP using the credentials you place into the setup. The account running the base monitor shouldn't have an affect on setting up AD in SQL Monitor.
There are some steps other steps you can try. The below documentation will guide you through adding a domain and user account manually via SSMS:

Keep in mind, this means altering the RedGateMonitor database. I would recommend taking a backup before you try and alter anything in there.
