Can I ask you to try uninstall SQL Dependency tracker and then re-install SQL Doc. Its possible that SQL Dependency tracker is blocking SQL Doc.
Please let me know if SQL Doc now appears.
Also I notice you are running an older version of SSMS. Are you able to update to the latest version of SSMS 18?

SQL Doc version: 5.1
The other redgate plugins (i.e SQL Search, SQL Test) are showing in ssms.
SQL Doc worked after I initially installed it two weeks ago, but disappeared coupled of days after.
tried to repair sql doc, uninstall sql doc and reinstall, no luck.
