How often do you patch your production SQL Server?

I have a persistent user who keeps pushing to upgrade all SQL Server instances to the current cumulative update. 

I say wait and at most do it yearly or quarterly?

What do you think?   What does your company do?

    In previous roles I've tended to take the approach of upgrading to service packs unless an active issue were to be fixed by upgrading to a CU.
