How you troubleshoot the "My server/query is running slow" call?

We're trying to improve how SQL Monitor helps you understand what problems a server might have right now. One scenario we have in mind is the call you might get: "My server/query is running slow". We want to understand what you typically do in those situations, what the best indicators of problems are etc.

We've got some ideas, but if you'd be willing to take a short call we'd really like your input so we can get a better understanding of the challenges.
