Hi David,
Unfortunately SQL Prompt needs to be reinstalled before activating in VS 2017 as extensions are not shared between Visual Studio versions. We know this is annoying and we're hoping to have a better solution in the future.

I had uninstalled then reinstalled SQL Prompt 7.1, but the SQL Prompt menu item still does not appear in VS 2017. Help. -

SQL Prompt 7.1 does not support VS 2017. Please install the latest version of Prompt, available here -

I wanted to let you know, your advice is not entirely correct. You don't need to install the latest version of Prompt to have VS 2017 support.
For everyone else in my situation, if you are using SQL Prompt version 7, you can upgrade to version 7.5 and still get VS 2017 support. My serial number for V7.1 still works (not trial). See release notes for V7.5 below.
