Custom alert using canned metrics?

Now that we have captured a good baseline, I'm trying to create custom alerts/email notifications that will fire when a specific canned metric exceeds a particular threshold. For instance, I want to be notified when the Disk Avg Read Time on a specific instance and drive letter exceeds 100 ms. Is this possible? I can't seem to find the right combination of 'custom' and 'out-of-the-box' functionality. Please advise. Thanks.
