Please check to see that you have no duplicate user names in the database. This shouldn't normally happen, as the server will not let you create two users with the same name, but it can happen if the database was restored from a backup and a user with the same name (but a different login name) was added into security and granted access to the database. In that case, you'll need to 'search and destroy' those duplicate users.
