Unfortinatly we have no way of automatically removing the menu from SSMS with the uninstaller. It is easy to remove it though (this text is pinched from the help).
In SQL Server Management Studio, to remove the SQL Prompt menu from the toolbar, do the following:
1) On the Tools menu, click Customize.
2) Drag the SQL Prompt menu from the menu bar, or right-click the SQL Prompt menu and click Delete.
3) Close the Customize dialog box.
Wish I could sort this bug out but I don't know anyway to do this automatically.

I'm currently beta testing v3. I attempted to uninstall "sqlprompt3_finalbeta.exe" so that I could do a fresh install of the "sqlprompt3_finalbeta_refresh.exe". My concern is that when I completed the uninstall of the product your "SQL Prompt" menu remained in SSMS. Whenever I clicked on an item under your menu it would say that the add-in wasn't available and I could remove the add-in items, but I could not find the option to remove your menu item. Once all the items under the menu had been removed your menu item was grayed out.
My concern is that I was going to have some other developers I work with test the product so we could make a formal request for it to be purchased. Unfortunately, if they decided to uninstall the product and the menu item remained this could be a problem. Can you please let me know how to remove the menu item in the event that your beta product is uninstalled.
