Well, I tried to come up with the 2nd scenario (beyond the details that I listed above) and I couldnt recreate the problem.
Note, I had rebooted since the earlier encounter. I will keep an eye out and try to record more detailed steps to recreate the situation if I come across it again.

additionally, if I do a very large select (eg 14+ish columns), I cant tab-complete columns after about the 14th column. (doesnt do anything)
eg
select c1,c2,c3,c4,c5,c6,c7,c8,c9,c10,c11,c12,c13,c14 <- can't apend any more columns with tab-complete
from table1
