Hi all,
Thanks for your feedback.
Do you also have SQL Refactor installed? If so, please try uninstalling SQL Refactor and then re-trying SQL Prompt 5.
Thanks,

and what if I need Refactor in order to work?
Thank you! -
Which features of SQL Refactor do you particularly need?
Eventually all features of SQL Refactor will become Pro features of SQL Prompt 5. Smart Rename and Split Table are already part of SQL Prompt 5.
If you let us know which features matter most to you, then we can prioritise the most popular for earlier Early Access releases.

The first time I received a message that SQL Prompt 5.0 could not start, Error nubmer 80131509.
I removed sql prompt 4 from the menu and restared SSMS. I received a crashreport:
Unable to cast object of type 'Microsoft.SqlServer.Management.SqlStudio.Explorer.ObjectExplorerService' to 'Microsoft.SqlServer.Management.SqlStudio.Explorer.ContextService'. The crashreport I sent to you.
After that SSMS started, SQL Prompt 4 is disabled and SQL Prompt 5 is there and seems to work. When I stop SSMS and restart it, I get the same error again.
I use Windows 7 64-bit Ultimate, SQL 2008-R2.
