I am still having this exact issue as of February 17, with SSMS 22.3.0 and the latest version of Red Gate tools downloaded and installed today. My SQL Prompt version shows as 11.3.5.21197. / comments

I have same issue where SQL Prompt doesn't work when using Microsoft Entra MFA SSMS 21.4.8 SQL Prompt 11.0.6.17341 Here is the relevant Red Gate log file entry: 2025-08-05 13:21:08.883 -07:00 [Warning] Failed to load server xxxxxxx.database.windows.net Microsoft.Data.SqlClient.SqlException (0x80131904): Login failed for user '<token-identified principal>'.
