I am using SSMS 18.12.1, but I actually got it to work by just uninstalling and reinstalling the latest SQL Prompt.  In the interim, I also installed SSMS 20, but SQL Prompt for now seems to be working in both SSMS 18 and 20.  Thanks for your help. / comments
