Hi, I also have this issue.   Microsoft Visual Studio Professional 2017 version: 15.9.40 SQL Prompt vesrion: 10.9.2.25020 SQL Prompt does not show up when a .sql file is opened in Visual Studio.   It works fine in SSMS. Thank you / comments
