Neil, I just noticed that there is a new version of SQL Prompt available. I installed it, and my problems starting QA are gone. Now my trial period for SQL Prompt is expired.

Neil, I tried the steps you suggested to fix the problem. Reseting the QA toolbar had no affect. Deleting the QA toolbar key from the registry fixed the error when QA starts up, but now I get a new error. As soon as I log in to QA, I then get a MS.Net error for SQL Prompt with the following detail.
